Partnerships for Better Health Act
Summary:
• Establishes a grant program administered by the Secretaries of Health and Human Services and Agriculture to fund integrated chronic disease prevention, nutrition, and social support services for underserved populations
• Grants of up to 3 years (renewable for 2 more years based on performance) are awarded to lead eligible entities such as federally qualified health centers, food banks, and nonprofit hospitals acting on behalf of local coalitions, with priority given to communities with high chronic disease prevalence, food insecurity, or located in rural or medically underserved areas
• Authorizes $15,000,000 per year for fiscal years 2027 through 2031
